Top 6 At-Home Fixes for Side Sleepers With Shoulder Pain in 2026, Reviewed and Ranked by a Physiotherapist

After 18 years treating shoulder and neck pain in side sleepers, these are the six fixes I'd actually recommend to my own family.

Five things side sleepers try for a sore shoulder, each with a star rating: stacking two pillows, a contour pillow, painkillers, a gel pack and massage, beside Dr. Donovan holding her blurred five-star #1 pick

If you sleep on your side and still wake at 3am with a shoulder that aches so much you have to roll onto your back...

If your arm goes dead, or fills with pins and needles, long before the alarm...

If you're sick of chasing a few hours of relief from heat packs, painkillers and a second pillow shoved under your head...

Then this is for you.

I've spent 18 years in clinic treating side sleepers with shoulder and neck pain. Every week someone sits down in front of me and lists everything they've already tried. The rolled-up towel tucked under the neck. The second pillow from the spare room. The heat pack at midnight. The massage booked every other Friday. The packet of tablets on the bedside table.

So over the past year I tested more than a dozen of them, exactly the way my patients use them. Not in a lab. In a real bed, lying on my side, asking one question. Does this fix the night, or just the next hour?

Some were downright useless. A few left the shoulder worse by morning.

But some stood out, and one in particular completely changed how my side-sleeping patients wake up.

Here are the pros, the cons and the real results of each.

But First, What's Actually Causing the 3am Shoulder Pain?

A woman lying on her side on a flat pillow at night, wincing, a red glow on the shoulder pressed beneath her

Most patients I see blame the mattress, a big day in the garden, getting older, or just 'sleeping funny'.

But the real cause is lying under their head. It's the pillow, and it happens in the same chain, every single night.

  • When you lie on your side, your shoulder is the widest part of you
  • A flat pillow makes no room for it, so it's crushed against the padding for six, seven, eight hours a night
  • Your head sinks into the middle and rolls you even further onto that shoulder, squeezing it tighter all night
  • The result? The 3am ache, the dead arm, pins and needles down to your fingers, and a stiff neck you're still rubbing at breakfast

In clinic, I call it the shoulder squeeze. And no amount of heat, tablets or extra padding stops it.

So the right fix has to deal with all of that, for the whole night.

Not just 'put some heat on it' or 'take something and go back to sleep'.

What I Tell My Patients to Look For in a Real At-Home Fix

Room for your shoulder
There must be deep cut-outs where your shoulder goes, so no padding presses on it. Your shoulder still rests on the mattress, the way it should. It just isn't crushed against a wall of pillow as well. Almost everything my patients try ignores this completely, and it's the number one reason side sleepers wake up sore.

A cradle that keeps your head where you put it
A dip in the middle with raised wings either side. Without it, your head slowly rolls your whole body onto that shoulder while you're asleep and can't do a thing about it.

The right height for your frame
Too low and your head drops toward the mattress. Too high and your neck bends the other way. Broad shoulders need more height than a small frame, so insist on two heights, like 8cm and 11cm, and use the one that fits you.

Comfort you'll actually use every night
If it's flat by winter or feels like lying on a brick, you'll stop using it, and a fix you don't use fixes nothing. It has to feel good on night one and still hold its shape on night three hundred.

#1 The Sleepsake Butterfly Pillow

Best Overall Relief, and the Only One That Stops the Shoulder Squeeze

5/5

Stocked by physios

The Sleepsake Butterfly Pillow

The only option I've tested that combines all four.

✅ Deep shoulder cut-outs on both sides, so no padding presses on your shoulder
✅ A head cradle with raised wings, so you don't roll onto it at 2am
✅ Two heights in one pillow, 8cm and 11cm (just turn it around)
✅ Soft on night one, still holding its shape on night three hundred

This isn't another contour pillow with a gentle wave in it that's flat by spring.

It's CertiPUR-US certified memory foam with five shaped zones working together all night. A central cradle for your head, a raised neck contour, deep shoulder cut-outs, raised side wings, and arm channels at each end so your arm has somewhere to go too.

My side sleepers with the worst shoulders feel the difference from the very first night. Within days, most tell me they're sleeping straight through, waking without the dead arm, and putting the painkillers away for good.

It's the pillow I recommend to every side sleeper who walks into my clinic with a sore shoulder. I keep one in the treatment room so they can lie on it before they leave, and I sleep on one myself.

"No more getting up at 4am to sit in the recliner to relieve his discomfort." Narelle Holton, verified buyer

  • Trusted by 20,000+ side sleepers and rated 4.8 from 700+ reviews
  • Five shaped zones built around the side sleeper's shoulder
  • 100-night trial, with every dollar back and return postage covered if it's not for you

Bottom line: The only fix that stops the shoulder squeeze all night, instead of masking the ache for an hour.

#2 Massage

Best for Loosening Up (But Undone by Morning)

3/5 Massage

I'm a physiotherapist, so believe me when I say a good massage feels wonderful. You walk out warm, loose and a little lighter than you walked in.

But watch what happens that night. You climb into bed on the same flat pillow, in the same position, for the same eight hours. Your shoulder spends the night crushed against the padding again, and by morning every bit of that work is undone. The next appointment is a week away.

One reader put it perfectly. "I started going for massages twice a month... That helps temporarily to relieve the headaches, but my neck is still stiff every morning." Melissa L., verified buyer

And at roughly the cost of a nice dinner out, every single time, it adds up fast.

  • Feels wonderful and loosens you up for the day
  • An hour of genuine time out for yourself
  • Costs about as much as a dinner out, every session
  • Undone by a single night on a flat pillow
  • No help at all at 3am, when the shoulder actually hurts

Bottom line: Feels great on the table, gone by the pillow. Nobody can book a massage for 3am.

#3 Gel Packs and Heat Packs

Best for Soothing the Ache Once It's Already Woken You

3/5 Gel Packs and Heat Packs

So many of my patients keep one in the freezer or the bedside drawer for exactly this. At 3am, that soft weight on a sore shoulder feels genuinely good.

But look at when you're using it. You're already awake. You've already spent four hours crushing that shoulder. You hold the pack on it, you drift off, it goes cold, and you lie straight back down on the same flat pillow, in the same position, until the ache wakes you again.

It soothes the shoulder for twenty minutes. It does nothing about what your shoulder is lying on for the other seven hours.

  • Cheap, and genuinely soothing in the moment
  • Easy to grab in the middle of the night
  • Only helps once the ache has already woken you
  • Loses its heat or its chill within twenty minutes
  • Zero effect on how you lie for the rest of the night

Bottom line: Feels nice, but not enough to change a single night. You ease the ache, then lie right back down on it.

#4 Painkillers

Best for Getting Through a Rough Night (Not for Every Night)

2.5/5 Painkillers

So many side sleepers keep a packet on the bedside table, and on a bad night it can take the edge off enough to get back to sleep.

But a tablet only masks the ache for a few hours. It doesn't change how you're lying for the other eight. Your shoulder is still pressed into flat padding, your head is still tipping you onto it, and the ache is waiting for you the moment it wears off.

Night after night, it stops being a fix and becomes a habit. Nobody wants to swallow something every single night just to sleep on their side, and none of my patients do.

  • Can take the edge off a rough night
  • Probably already in the bathroom cabinet
  • Masks the ache for a few hours while the way you lie stays the same
  • Does nothing about what your shoulder is pressed against
  • Not something anyone wants to need every night

Bottom line: It can mask a bad night. It will never give you a good one.

#5 Stacking Two Pillows

Best for a Hotel Room When There's Nothing Else

2/5

It's the first thing almost everyone tries, and in clinic it's the one I see backfire most. One pillow feels too low, so you grab a second one and stack them. Your head comes up, and for a minute it feels better.

But two flat pillows are still flat. There's more padding and still nowhere for your shoulder to go, so now there's simply more pillow pushing back against it. The stack is usually too high as well, so your neck bends the other way all night.

And two loose pillows never stay put. By 3am they've slid apart, and you're half awake, folding one in half and punching the other back into shape.

  • Free, and already on the bed
  • Lifts your head a little
  • More padding pressing on the shoulder, not less
  • Slides apart and squashes flat by midnight
  • Often too high, so your neck bends the other way

Bottom line: More padding was never the answer. It just squeezes the shoulder harder.

#6 Contour Pillows

Best for Back Sleepers (Not Your Shoulder)

2/5 Contour Pillows

These are the pillows with a gentle wave along the top, sold everywhere as ergonomic or cervical. Of everything on this list, it looks the most like a real answer, and my patients are always surprised to see it last.

For back sleepers, the wave does its job. It fills the curve of your neck and holds your head level.

But roll onto your side and look at what's under your shoulder. Nothing has changed. There are no cut-outs, so your shoulder is still jammed against a solid block of foam, with your head now propped up on top of it. The neck feels supported. The shoulder is just as squeezed.

I've put it last for one reason. It's the fix people stop looking after. They think they've solved it, so they live with the 3am shoulder for years. I see it every week: a good contour pillow, a better-looking neck, and the same dead arm.

  • Supports the neck well if you sleep on your back
  • Better than a flat pillow for your neck
  • No cut-outs, so your shoulder is still jammed against solid foam
  • Props your head up on top of a squeezed shoulder
  • Feels like the answer, so people stop looking for the real one

Bottom line: Better than flat. Still squeezes the shoulder.

Like a lot of things on "the intenet" i was very sceptical. However, the relief was instant and I have been sleeping well without any shoulder pain. Not sure how it actually works, as im sleeping in my sore shoulder and waking up with a fully functional shoulder.

I bought this for my husband as he has neck and shoulder issues that were affecting his sleep. The butterfly pillow brought him instant relief the very first night he used it. No more getting up at 4am to sit in the recliner to relieve his discomfort. He loves it.

I have had right shoulder pain from a rotator cuff injury for over five years, and even surgery did not fix it. I could not sleep on my right side without pain. But this pillow has actually worked. I was able to sleep comfortably on my right side from day one with this pillow. I recommended it to a friend who has also purchased one for herself and one for her Father.

Bought this for my 85 yr old husband... Sleep for him had become a nightmare and he always woke up exhausted from pain and lack of sleep. From the first night of using this pillow, he finally slept through. No more of him doing his Mick Jagger impersonations, - rocking and rolling and groaning all through the night! Now we both get a good nights sleep.

It took a few nights to get used to as normally I slept with my arm under me... Now I sleep thru most of the night and I'm waking up feeling way better. No crushed shoulders... Well worth the money and I dont get hot with this pillow I love how it cradles my neck.

In 13 yrs I have bought that many pillows I could open a pillow shop and none of them worked. I'd fight with pillows each night like a Kung Fu Panda!!... The first couple of nights I slept like a baby. I went from Kung Fu Panda to sleeping beauty.

Have had shoulder pain waking me for over two years and this has stopped it... I don't know how, but it works. I've just bought one for my dad as he has severe shoulder pain that wakes him frequently during the night...

I bought 2 pillows, one for myself and the other for my partner... My partner took 2 nights to adjust and he is equally hooked by the butterfly pillow... My partner has very wide shoulders and he has suffered always with normal pillows, waking many times during the night... He now sleeps through the night he is less restless while sleeping...

I was sceptical. I've tried numerous pillows & nothing. This was my last chance. I still can't believe it, but it's working. No waking up in the middle of the night with shoulder pain. I'd recommend purchasing one & with the 100 day free trial, test it for yourself.

...for me these pillows have actually made me look forward to sleeping. They say it takes a week to adjust to a new pillow; for me it was one night. I was struggling with shoulder pain for three years and it worked like magic.

I was skeptical about this and was thinking it was a bit firm and high. I turned it around because I have a short neck and it was perfect. No sore shoulder or neck in the morning. I now think I'm going to be one of those people who travels with their pillow.

I liked the pillow right from the first night but, what has surprised me is, the longer I am using it the better nights sleep I'm getting. If I wake during the night it's not because I'm uncomfortable or have to adjust my pillow anymore :-)

The first few nights were getting accustomed to the recommended way to use. I have noticed a reduction in tossing and turning and some nights I wake feeling as if I haven't moved all night. The quality of my sleep has improved but most of all I love the comfort of the pillow and won't be going back to my old one.
